1. Select the left navigation option, Form Builder, to build a form.
Note: At a glance, see all of your existing forms and relevant data about them.
2. Click + NEW FORM to begin the form building process.
3. Complete the Add Form pane and click Create.
Form Name: This field assigns the title of the form. This must be unique.
Form Description: This is an internal field for notes/details about the form.
Group Assignments: Assigns the form inside of your Web Store. To learn more about Groups, see our Intro to Groups article.
Visibility Dates: Enables auto-ending a forms visibility in your Web Store. If no end date is set, the form will remain visible indefinitely.
Status: Default to Active. Archiving a form will remove it from the Web Store. It can be reactivated at any time. To learn more about archiving a form, see our How to Archive a Form article.
Responder List: Limits who can view/submit responses to a form. This feature leverages your current Web Store Import Lists. To learn more about Import Lists, see our Add an Import List article.
Report Frequency: Enables scheduled emailed reports. To learn more about Automated Reporting, see our Automated Reporting article.
Template: Provides a list of pre-crafted templates that are commonly requested. These can be customized during the form building process.
4. The Form Editor will open in a new browser tab.
5. To add Payments to your form, drag and drop the RevTrak Inventory Element to the desired location in the form.
6. Enter the SKU from the inventory item that you want to add to the form. Click Search. RevTrak Forms leverages your current Web Store Inventory Items for payments. To learn more about Web Store Inventory Items, see our Intro to Items article.
7. Click the results, in this scenario ‘Tshirt’.
Note: SKU Search will only find exact matches. Partial matches will not return search results.

8. The Inventory Item has been added to the form. Publish and its ready to handle submissions in the Web Store.
